In order to participate in the ClearOne Rewards program, ClearOne channel partners’ individual employees must register and create a personal account on the ClearOne Rewards promotion web site. With this account, participants can submit claims for sales of ClearOne products, view the status of claims they have submitted, redeem rewards and track the shipping or processing of orders for reward items.

Registration steps:

1. Participants must register on the promotion web site.
   To register for the program, participants will be required to complete and/or provide the following information:
   1. Accept terms and conditions
   2. Enter official business name, address, and country
   3. Enter name, title, email, phone number
   4. Select username and password
   5. Select password reminder question and answer
   6. Agree to receive ClearOne Channel Newsletters
   7. Agree to receive emails from ClearOne Rewards program
2. As proof of sale, the participant must fill out a claim on the website and submit an electronic copy of their customer invoice or fax a hardcopy to ClearOne Channel Marketing, fax number (801) 303-3337. The ClearOne Rewards website will ask for the following information about the sale of the eligible products:
   1. Date of sale
   2. Eligible products sold
   3. Quantity
   4. Invoice #
   5. End-User Company Name – For Distributors this will be a Dealer and for Dealers this will be an End-user.
   6. Copy of invoice
3. There is no limit on the number of claims and points that can be earned.
4. All US participants who earn more than 12,000 points ($600) will be required to submit a completed W-9 form.  If a participant spends $600 or more in a calendar year that individual will receive a 1099 form from ClearOne.
5. Participants will be able to view and track historical claims submitted. Once a claim has been approved an email will be automatically transmitted to the participant who submitted the claim. Any comments added by the program administrator will be transmitted to the participant as part of the email. Participants will be able to track the claims they have submitted by logging into the program web site and selecting, track claims.
6. Allow 2 weeks from the time a claim is submitted to receive approval for the claim.
7. Incentive points do not expire but may be forfeited. Participant accounts that remain inactive for 12 months will be disabled and points will be forfeited.
8. This incentive program is void wherever prohibited or restricted by law.
9. ClearOne reserves the right to modify or withdraw this incentive program and rewarded points at any time. We reserve the right to make changes to this Agreement, or the guidelines or rules incorporated herein, at any time. Your continued use of our Site will signify your acceptance of any revised terms to this Agreement. Therefore, you should regularly visit this page to make yourself familiar with our current Agreement. This Agreement may not otherwise be amended, except in writing, by both you and ClearOne. Unless otherwise stated, all amended terms to this Agreement shall be effective immediately. User Information: "User Information" is defined as any and all information or data you provide to ClearOne or other users during the Registration or any other process, and through any interactive feature of the system, including email. You represent and warrant that your User Information (i) is not false, inaccurate, incomplete or misleading; (ii) is not fraudulent and does not involve illegal, counterfeit or stolen items; (iii) does not violate any law, statute, ordinance or regulation (including without limitation those governing export control, consumer protection, unfair competition, anti-discrimination or false advertising); (iv) does not contain any viruses, Trojan horses, worms, time bombs, bots or other computer programming routines that are intended to damage, detrimentally interfere with, surreptitiously intercept or expropriate any system, data or personal information.
10. Breach: Without limiting other remedies, we may limit your activity, immediately remove your registration and rights to point accrued, temporarily suspend, indefinitely suspend or terminate Your Account and refuse to allow you to continue participation in the ClearOne Rewards Program if: (a) you breach this Agreement or the guidelines or rules it incorporates by reference; (b) we are unable to verify or authenticate any information you provide to us; or (c) we believe that your actions may cause undue financial loss or legal liability for you, our users or ClearOne.
11. General: This Agreement shall be governed by and construed in accordance with the laws of the State of Utah. The United Nations Convention on Contracts for the International Sale of Goods shall not apply to this Agreement. You and ClearOne agree to submit to the personal and exclusive jurisdiction of the courts located within the County of Salt Lake, Utah. Headings are for reference purposes only and in no way define, limit, construe or describe the scope or extent of any section of this Agreement. If any part of this Agreement is deemed invalid or void, that part of the Agreement shall be deemed severable and shall not affect the validity or enforceability of any of the remaining conditions. ClearOne’s failure to act with respect to a breach by you or others does not waive our right to act with respect to subsequent or similar breaches.

Eligible Participant
All ClearOne Dealers and Distributors in North America are eligible to participate in ClearOne Rewards. ClearOne Partners have the right to restrict use of the ClearOne program with their own employees. ClearOne Dealers are any organization that sells directly to end users and ClearOne Distributors are any organization that sells to a reseller or dealer.

Can this promotion be combined with other offers?
ClearOne Rewards cannot be combined with any other ClearOne promotion or spiff.
